If you choose to upgrade to CS150 you will now be able to go to a specialist with the services needed at the same cost as listed on the benefits page. You will now save more than 25%! These premiums are quoted below:   

CS150 (prestige plan) – Employee - $13.88

Employee + 1 dependent - $26.38

Employee + family - $35.98

You now have the freedom for family members to go to a different dentist. Remember that the dentist has to be on the provider list but each member in one family can choose their own dentist.
